Worker Crushed to Death, Construction Contractor Cited

CALIFORNIA – According to an online news report at, a contractor in California was cited for serious violations of safety after an employee died after being crushed to death on a construction site in San Rafael, California.

A construction contractor in San Rafael, California at a construction site was cited for serious violations of safety after an employee died by being crushed to death.

According to the California Occupational Safety and Health (Cal/OSHA), it issued $26,540 worth of citations to a company based in Concord, California, per a press release.  

On September 18, 2018, the accident occurred when two employees were installing and framing a shear wall on the 3rdfloor of a residence that was under construction.  A worker went to obtain plywood from a pile of twenty six panels that were leaned against a wall. Another construction worker discovered the employee’s body twenty minutes later because he was crushed to death by the stack.

As a result of the accident, Cal/OSHA issued citations to the company for 8 violations.  According to Cal/OSHA, “[i]n addition to the serious accident-related violation for failing to properly secure the vertically stacked plywood, inspectors also cited the company for serious hazards relating to a failure to guard an opening on the house’s second floor, failure to protect workers from impairment hazards on exposed reinforcing steel projections, and failure to protect workers from falls of 15 feet or greater.”

Cal/OSHA also issued the company with a “regulatory citation” for its failure, as alleged, to get a permit for construction related work.  The company was further cited for other violations regarding it alleged failure to “evaluate hazards, protect workers from loose rock or soil, or provide handrails and stair rails,” per a press release issued by Cal/OSHA.

There are no further details on the accident at this time.
